As an NDIS Support Coordinator you will be:
- Empowering clients with a voice and choice of plan implementation that meet their goals
- Developing and recording progress of goals for clients that are specific, measurable, achievable, relevant and timely (SMART)
- Strengthening and enhancing a client’s capacity to coordinate supports, self-direct and manage supports along with community participation
- Supporting clients in liaising with a plan manager to establish appropriate claim categories and funding allocation
- Developing and establishing effective working relationships with community organisations (mainstream and disability) aligned to client outcomes
- Encouraging clients to exercise choice and control to access a range of formal and informal mainstream and community-based support, by identifying opportunities for innovative service delivery
- Promoting Inebura Services in the community and generating new enquiries through networking activities, and attending community and social events related to disability service industry
Qualifications, Skills and experience required:
- Tertiary qualification in Social Work or relevant field
- Exceptional customer service and problem-solving skills
- Full Australian Driving licence and access to Car (essential)
- Current and valid Australian Federal Police Check & Working with Children Check or NDIS Worker Screening
- Excellent verbal and written communication, including a high standard of report writing
- Comprehension of the NDIS legislation and rules including provisions relating to reasonable and necessary supports
